All applications must have Sections 1 – 4 filled in correctly. Any application not filled in correctly will be returned to the student. Students should hand in their completed application to their Homeroom teacher or the Attendance Office. Any student that was on the program at the end of the 2017-2018 school year will remain on the program until October 8, 2018. If a NEW application has not been handed in for the 2018-2019 school year by October 8, 2018, the student(s) will be removed from the program. Households that were notified that they qualify under Direct Certification do not need to apply.
Please notice that you are able to list all students in the household on one application. Please make sure you write the correct school each child attends (Wingate, Port Matilda, Howard, Mountaintop, or Middle/High School) in the line beside the student’s name.

Please remember that the income you report should be your Gross Income, not Net Income (unless you are self-employed).
